Officer’s Quarterly Report

If you are currently serving as a Distinctive Religious Group Leader, At-Large Officer, or currently lead a Civilian Open Circle, you are required to fill out the following form to submit your quarterly report. The report should be filled out NO LATER THAN the 1st day of the month following the Equinox or Solstice (ie. Yule/Winter Solstice Report is due 01 JAN). Thank you.
